We believe that each individual who receives our services deserves dignity and skillfully managed care. Rocky Mountain Hospice provideshospice care in patients' homes, skilled nursing facilities, hospitals, assisted living centers, residential care facilities, or whatever the patien calls "home."
The team at Rocky Mountain Hospice provides quality care for patients and families by fostering individual choices for dignity and comfort.
WHAT IS HOSPICE? Hospice is a program to assist individuals, their families and/or caregivers acheive the best quality of life during a life-limiting illness. Hospice treatment includes physical, emotional and spiritual care. Hospice focuses on patient comfort instead of curing the underlying disease or condition. Hospice patients are encouraged to participate in activities and functions they enjoy. Hospice is for children, adults and the elderly.
THE GOALS OF ROCKY MOUNTAIN HOSPICE ARE: •Supporting patients and their loved ones throughout this important transitions time.
•Fostering peace in the dying process
•Promoting open discussions for all care options and supporting the patient's choice.
TO BE ELIGIBLE FOR HOSPICE CARE, AN INDIVIDUAL MUST HAVE: •A life-limiting illness causing a general decline.
•An estimated life expectancy of six months or less.
•A treatment goal of comfort rather than cure.

•Health insurance
•Private pay
•Grants and private donations
WHAT'S INCLUDED IN YOUR HOSPICE BENEFIT? •Emotional, spiritual, finantial and bereavent support for patient and family.
•Home health aides and homemaker assistance services.
•24-hour on-call doctor and nursing availability.
•Required medical equipment and medications and training in their use.
•Mediacl and incontinent care supplies.
•Short-term inpatient care required to manage symptoms.
•Physical, occupational and speech theropy as needed to enhance the quality of life.
•Counseling services.
•Assistance with legal documents (wills, power of attourney, advance medical directives, etc.) funeral planning, and accessing other community resources.
